  5. House calls are available (within a 20 mile service radius)

  7. Well-baby checks and youth sports physicals are included

* Vaccinations included with membership: Fluvirin & Fluzone (annual flu), Hiberix, Energix (Hep B), Twinrix, Havrix (child's Hep A), Daptacel (DTap), Menactra, Polio, Adacel (TDap), Varivax (Chicken Pox), and Pnuemovax / Zostovax (Shingles).   Travel vaccinations, Gardasil immunizations, and other recommended vaccines not listed are available upon request and are billed separately.
